Your initial appointment will consist of a comprehensive exam, using an intra-oral camera and x-rays, to diagnose and develop a treatment plan. Occasionally, cleanings can be performed the same day. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your first visit:

• Your x-rays if taken in another office in the last 5 years. If another dentist has taken
  x-rays, as a courtesy we can request they forward them to our office.
• A list of medications you are presently taking
• If you have dental insurance, please bring your insurance card so we can help expedite reimbursement.
